Background & Aims
Painful Diabetic Peripheral Neuropathy (PDPN) poses a significant challenge in diabetes management, necessitating effective interventions with minimal side effects. Topical capsaicin, derived from chili peppers, has emerged as a promising therapeutic option due to its interaction with the transient receptor potential vanilloid 1 (TRPV1) channel. This scoping review aims to explore the concentration variance of topical capsaicin and its efficacy in mitigating PDPN symptoms, addressing the need for standardized guidelines in its application.
Methods
A systematic search was conducted across major databases, including PubMed, Embase, and Cochrane, identifying studies that investigated the impact of different concentrations of topical capsaicin on PDPN. Inclusion criteria encompassed randomized controlled trials, observational studies, and systematic reviews. Data extraction involved details on study design, patient demographics, capsaicin concentrations, application protocols, adverse effects, and outcomes related to pain relief.
Results
The scoping review revealed significant heterogeneity in the concentrations of topical capsaicin used in various studies. Studies advocated for both lower concentrations for gradual desensitization and higher concentrations for enhanced efficacy. Adverse effects, such as burning sensations and erythema, were dose-dependent, emphasizing the importance of striking a balance between concentration and tolerability. Standardized protocols for capsaicin application remain elusive, highlighting the need for future research to establish optimal dosage guidelines.
Conclusions
The review underscores the imperative for establishing standardized guidelines on the concentration of topical capsaicin for PDPN management. The concentration-dependent effects observed in the literature necessitate a nuanced approach, considering both therapeutic efficacy and patient tolerability. While lower concentrations may minimize discomfort during application, higher concentrations may offer superior pain relief. Individualized treatment plans, considering patient-specific factors, are crucial for optimizing the balance between efficacy and safety. This scoping review provides valuable insights into the concentration variance of topical capsaicin, serving as a foundation for future research and contributing to the evolving landscape of alternative therapies for PDPN.
